(CHOICES has previously been referred to as Midland Care Advocacy.)

A variety of services are provided through the Midland CHOICES program. We expect the number of clients will continue to grow in the Care Management and Aged and Disabled Medicaid Waiver programs due to the growing client need, agencies recognizing the benefit of the service to the client, overall community agency cooperation, and the ability to better staff the programs.

Care Management
Care Management Services are provided by Nebraska's Area Agencies on Aging. The certified Care Managers arrange for coordinated care in the home. We can help you find assistance for the services you need, whether they're needed daily, weekly, or monthly.
There are solutions to some of the problems that face today's adults. Care Management can help you get the assistance you need at an affordable cost.
Most of us know there is help for a particular situation, if we just knew where to look. That's where Care Management Services can help. Our experience is your benefit.
We know which services are designed for you and which aren't. But we don't make decisions for you; we simply help you make better, more informed decisions.
When you work with Care Management offices, you'll receive the help you need--whether it's information you'll use once a month, or a service you'll use every day.
The Care Management Office is at your local Area Agency on Aging. Senior Care Options and Medicaid Waivers are also part of the CHOICES program that can assist you in accessing the right services.

Care Management FAQ
Dealing with too many agencies that provide care for older adults is confusing. How can I keep track of who does what?
A Care Manager knows what's available in your community and which agency does what. The Care Manager can cut through the red tape and speed up your request for services by coordinating with other agencies.

How do I know that Care Managers will have a clear idea of what my needs are?
When you contact Care Management, you'll receive a thorough and personal evaluation of your situation by a qualified care manager. This helps us determine what type of assistance you'll need. Care providers are then contacted by your Care Manager according to the types of services you have requested.

I can't always depend on those closest to me for help. Why should I trust you?
Care Managers don't have a vested interest in any one service area. As a result, you can rely on our expertise because our interest is in YOU. They can assist you in making the right decisions.

I'm on a fixed income. How can I afford this?
The cost of Care Management won't make you feel like you're in over your head. The fees are affordable and based entirely on your level of income. Many in-home services are also affordable

My needs change from month to month. Is this program flexible?
Because Care Managers are familiar with a variety of services available to older adults, you'll have a number of options. Whatever types of services are recommended, you control what happens. You decide.

Senior Care Options
Senior Care Options is available for older Nebraskans who are considering admission to a nursing home and want Medicaid to pay for the needed care.

Senior Care Options:
-Helps older Nebraskans find the right care at the right time.
-Has information about choices to allow older Nebraskans to live independently for as long as possible.
-Is designed to save taxpayer dollars by making Medicaid efficient.

Senior Care Options FAQ
What if I want to live in an assisted living community?
Assisted living is considered a community-based service within Senior Care Options (SCO). Your SCO counselor can help you identify assisted living communities in your area and explain the availability of Medicaid funds for eligible individuals.

What if I need Medicaid to pay for my nursing home care?
Medicaid funds may be used for nursing home care, or for home and community-based services.
Anyone can apply for Medicaid at their nearest Nebraska Health and Human Services office. Senior Care Options (SCO) becomes a part of the process when a person aged 65 or older requests Medicaid payment for nursing home care.
Health and Human Services staff determine financial eligibility, and SCO counselors evaluate the need for nursing home care.

What if I have health concerns, but I don't want to move to a nursing home?
This is where Senior Care Options can be especially helpful.
Even if the Senior Care Options evaluation show that you do qualify for nursing facility care, you may still be able to stay in your own home and receive services.
Your SCO counselor will help you determine the specific types of assistance you need to remain independent, such as: housekeeping, home-delivered meals, personal care, respite, or adult day health care.
Your SCO counselor will then help you explore possibilities for funding those services. Some of the services may be paid for through Medicaid.

How would the Senior Care Options program evaluate my health?
The Senior Care Options counselor consults with you and your family members; hospitals and nursing home staff; physicians and other service providers to learn:
-How much assistance is needed in performing activities of daily living, such as dressing, walking, eating, and personal care.
-Whether there are concerns about your safety, behavior, or memory.
-If your health condition requires professional nursing services.

If I plan to pay for my own care, does Senior Care Options apply to me?
State law requires SCO evaluations only for people involved with Medicaid. However, many people who first use their savings to pay for nursing home care later apply for Medicaid.
It's important that you have this information now. If you need Medicaid payments after you've lived in a nursing home for awhile, SCO must evaluate your needs then.

Can Senior Care Options help me if I am not eligible for Medicaid and don't need to go to a nursing home?
Yes. The Senior Care Options program is operated by your local Area Agency on Aging. Care management, needs assessment, information and referral, and a variety of other services are available to older Nebraskans (65+) through their Area Agency on Aging.
