Advocate and Provide Quality Services to Older Adults and their Families

CHOICES is a program that helps seniors find in-home services and care providers to allow them to live independently for as long as possible, postponing nursing home placement.  Our CHOICES Coordinators include R.N.’s, Social Workers, and other human services professionals.  We offer:

· An assessment of areas where help is desired, what assistance is already being provided and to determine what programs the person may be eligible for.

· Information from the Coordinator on what is available and assistance in setting up chosen services if the client prefers, including locating caregivers and financial assistance.  The client makes all decisions in regard to what will be done.

· Monthly visits or calls to the client to check for satisfaction  with services, assist with problem-solving, advocate for the client when needed, and continue to provide client and family information as their situation changes

 

The client may receive services in his/her own home or at an assisted living facility.  Depending on the program(s) that the client is eligible for, services may include the following:

· Housekeeping

· Home Delivered Meals

· Meal Preparation

· Adult Day Care

· Supervision/Respite Care

· Assisted Living

· Lifeline

· Transportation to medical appointments or essential shopping

· Reduced co-pays on medications

· Nursing Visits

· Modifications to make the home more accessible

 

Cost of Service

· CHOICES Coordinator—Most clients receive the services of a CHOICES Coordinator at no cost.  There may be a sliding fee scale for those with higher incomes.  Payment is on a contribution basis and is welcomed but not required. 

· All other services—Many clients qualify for assistance through state/federal programs and receive other services at no cost.  However, clients with higher incomes may not qualify for financial assistance.  The cost of a service is always discussed and approved by the client/surrogate decision maker before it is provided.
